2025 FCDTC Membership Renewal - Family
Description
This form serves as the new waiver that all class participants will now be required to sign when registering for classes. Every FCDTC member MUST also have one on file. A new waiver must be signed each year.
WAIVER
The undersigned hereby releases and forever discharges Forest City Dog Training Club, Inc. its owners, members, officers, employees, agents, heirs and assigns, from any and all actions, rights, causes of actions, claims and demands for or upon which have been, or which hereafter may be, sustained by the undersigned and the undersigned’s dog(s), as a result of said dog and owner participating in activities at the Club building or at activities conducted by the said club for the year.
The undersigned understands that during the said activities both dog and owner may engage in events which may result in injury to the dog or the owner. This release extends and applies to, and also, covers and includes, all unknown, unforeseen, unanticipated, and unsuspected injuries, damages, losses and liabilities, and the consequences thereof, as well as those now disclosed and known to exist. The provision of any state, federal, or local law or statute providing in substance that releases shall not extend to claims, demands, injuries or damages which are unknown or unsuspected to exist at the time, to the undersigned, are hereby expressly waived. The undersigned has carefully read the foregoing and knows the contents thereof and signs this release of their own free act and deed.
The member acknowledges the emailing of the Recall, annual notifications to AKC of members, meeting notices or any other club notifications to my/our email address as listed on the membership list. I/We hereby release Forest City Dog Training Club Inc., from any liability should notification be received late or not received at all by members due to circumstances beyond the Club’s control It shall be the responsibility of Club member(s) to keep the secretary informed of any change in your email address.
